Interview with Amy Dubey, OTR

Amy Dubey, OTR - pediatric OT and adult/pediatric TBI, sensory integration; 10 years’ experience, currently in private practice; trained in sensory integration, iLs and Interactive Metronome; company is Therapy Solutions, in Wolverine, MI.

What client population(s) do you work with? I work in a rural area. I’m the only private practice pediatric OT in the Northern Michigan region, so I handle a diverse case load comprised mostly of pediatric SPD and pediatric/adult
Mild Traumatic Brain Injury (MTBI).

How does iLs complement your practice?
It brings a neurological connection that you don’t get through repetitive tasks. I like the way it coordinates the rhythm, timing, auditory, visual and coordination in one session. And that it can be done at home, because we know the frequency is so important. The ease of use of iLs for families is a big plus – I get more compliance using iLs than I ever did using OT activities.

Do you use iLs Receptive and Interactive (Expressive) Language programs?
Yes, most of the time I work in conjunction with an SLP as a co-treat so we coordinate the Total Focus receptive program and the Interactive Language Program (ILP). For those with MTBI, we begin the ILP with humming and auditory figure ground exercises; simultaneously, we work on their fine/gross motor and auditory processing with the Total Focus. Children with SPD usually require some OT and Total Focus work to be able to sit still long enough for the SLP to use her tools.

Can you give some case examples that reflect the results you’re seeing?
Yes, we’re in the middle of a few very exciting ones right now: a child with Down Syndrome, another with SPD and a man with MTBI.

The 11-year old girl with Down’s has been in therapy for most of her life. Since we began working together 8 months ago, she’s made remarkable gains in her gross motor skills, reading, speech and socialization skills. Her mother just reported she has begun to self-initiate play for the first time in her life.

Also, I have been working with a 6 year-old who has sensory and auditory processing deficits: SPD, in constant motion, constantly seeking input, extremely poor visual attention, no eye contact, poor auditory processing; SLP worked with her for a few months and got minimal results. The young girl couldn’t sit still to use the speech tools.  We began a combination of SLP and OT using iLs and sensory input. She can now attend to get 45 minutes of speech therapy; amazing changes in comprehension, attention, expressive skills; mom and dad are elated and are considering an iLs home unit.

I’m also very happy about an MTBI client: a young man in 20’s, who had multiple TBI’s over 5 years ago. He’s wheelchair constrained and about 25 sessions into the iLs Sensory Motor Program. He started out depressed, had a bad attitude and zero motivation. But now he’s seeing some real results and his attitude is changing. He can stand with contact guard with good balance while leaning against something; his left eye (which tended to roll back, out of control) now can be controlled, more often straight ahead; his socialization is more appropriate, and his rate of speech is smoother instead of slow and choppy. He is feeling good about himself. After 5 years of therapy and no significant gains, this is really exciting to witness!

iLs Practitioner Training Schedule 2011

Please note the updated 2011 Practitioner Training Course (PTC) dates in the left column. We have now added dates and locations throughout the rest of the year.

If you have already completed your Practitioner training you are welcome to attend a refresher course.  The fee for attending a course as an iLs Associate is $25.


Testing Your Bone Conduction Headphones

Two of our most frequently asked questions are about the functioning of the bone conduction transducer in our headphones. Is it working?  How do I test it? As of Spring 2010, iLs began loading a BC Test in all Focus units, which you can play to test the BC level. This is done by finding ‘BC Test’ (under "Playlists") on the iPod, clicking "Play" and – gently, but firmly, squeeze with your index finger on the bone conductor and your thumb on the headband. The vibration should be unmistakable, but will fluctuate as the low frequencies within the music fluctuate.

Last week,
iLs held its first ever web conference for iLs Associates, hosted by Ron Minson, MD. The topic of the webinar was "Challenging Cases" and focused on answering some of the cases presented by our clients. A recording of the webinar is archived in the Associate Resource Center of the iLs website.

The webinar series schedule for 2011 will be announced in March.
